The short answerWeight loss injections are prescription medicines, mainly GLP-1 drugs such as semaglutide and tirzepatide, that lower appetite and help adults with obesity or weight-related health problems lose weight. In large trials, people taking them with diet and activity changes lost far more weight than people on a placebo. Vitamin B12 and lipotropic shots are a different thing: they are not weight-loss medications.
These shots show up everywhere now, from doctors’ offices to social media ads, and not all of them are the same. Some have years of trial data and a detailed FDA label. Others are vitamin shots with a weight-loss name. Dr. Melanie Espinet, a board-certified family physician in Lutz, explains the types, what the evidence shows, who qualifies, what to watch for, and what they cost.
What are weight loss injections?
They are prescription medicines given as a shot under the skin, used alongside healthier eating and more activity. The NIH’s NIDDK lists the medicines approved for long-term weight management. Four of them are injections: semaglutide (Wegovy), tirzepatide (Zepbound), liraglutide (Saxenda), and setmelanotide (Imcivree), which is used only for specific rare genetic conditions.
The first three belong to the GLP-1 family. They copy a gut hormone that helps control appetite. If you want the biology first, start with what GLP-1 is and how GLP-1 medications work.
Types of weight loss injections
When people look for injections to reduce weight, they usually find two very different groups: prescription medicines tested in large trials, and nutrient shots. Here is how they line up:
| Injection | What it is and how often |
|---|
| Semaglutide (Wegovy) | GLP-1 receptor agonist, once a week. Labeled for weight management and heart risk reduction |
|---|
| Tirzepatide (Zepbound) | Acts on two hormone receptors, GIP and GLP-1, once a week. Labeled for weight management and for sleep apnea in adults with obesity |
|---|
| Liraglutide (Saxenda) | GLP-1 receptor agonist, once a day. Labeled for weight management |
|---|
| Setmelanotide (Imcivree) | Once a day, only for specific rare genetic conditions |
|---|
| Vitamin B12 shots | A vitamin. Not a weight-loss medication |
|---|
| Lipotropic shots | Blends of nutrients. Not a weight-loss medication |
|---|
Are B12 and lipotropic shots weight loss injections?
No. Vitamin B12 is a nutrient your body needs, and lipotropic shots are nutrient blends. Neither one is on the NIH list of approved weight management medications. MeliOra offers Revital B12 and Lean Ease Lipo-C as optional wellness shots, and we are honest that they are not a weight-loss treatment. For the evidence, read whether B12 shots help with weight loss.
How do weight loss injections work?
GLP-1 weight loss injections work mainly on appetite. According to NIDDK, semaglutide and liraglutide mimic GLP-1, a hormone that targets areas of the brain that regulate appetite and food intake, and tirzepatide mimics both GIP and GLP-1. The Wegovy and Zepbound labels say these medicines lower calorie intake, likely by affecting appetite, and that they slow stomach emptying.
Your own GLP-1 lasts only minutes. These medicines are built to last much longer: semaglutide’s half-life is about one week and tirzepatide’s is about five days, which is why both are taken once a week. Doses start low and rise in steps, so your body has time to adjust.
Do weight loss injections work? What the trials show
Yes, for many people, when the injection is paired with diet and activity changes. These are the average results from three large randomized trials in adults with excess weight who did not have type 2 diabetes:
| Trial (length) | Average change in body weight |
|---|
| STEP 1: weekly semaglutide vs. placebo (68 weeks) | Down 14.9% with semaglutide, compared with 2.4% on placebo |
|---|
| SURMOUNT-1: weekly tirzepatide at three doses vs. placebo (72 weeks) | Down 15.0% to 20.9% depending on dose, compared with 3.1% on placebo |
|---|
| SURMOUNT-5: tirzepatide vs. semaglutide, head to head (72 weeks) | Down 20.2% with tirzepatide and 13.7% with semaglutide |
|---|
These are averages, so some people lose more and some lose less. NIDDK adds three practical points. Most weight loss happens in the first 6 months. If you have not lost at least 5% of your starting weight after 12 weeks on the full dose, your doctor will probably suggest stopping. And you will probably regain some weight after you stop. The labels describe these medicines as treatment to reduce weight and maintain the loss long term.
What is the best weight loss injection?
There is no single best weight loss injection for everyone. In SURMOUNT-5, a large head-to-head trial, tirzepatide led to more weight loss than semaglutide over 72 weeks. That trial was open-label, meaning participants knew which drug they were taking, and it was funded by the maker of tirzepatide.
Average weight loss is only one part of the choice. The right injection also depends on how you tolerate it, your health history, the conditions it is labeled to treat, and the monthly cost. For example, Wegovy is also labeled to lower the risk of heart attack, stroke, and cardiovascular death in adults with established heart disease and obesity or overweight, and Zepbound is also labeled for sleep apnea in adults with obesity. Our semaglutide vs. tirzepatide comparison goes deeper, and Zepbound vs. Wegovy compares the two weight-management brands.
Who qualifies for weight loss injections?
The weight-management labels cover adults with obesity, or adults with overweight plus at least one weight-related health condition. NIDDK puts that in numbers: a BMI of 30 or more, or 27 or more with a problem such as high blood pressure or type 2 diabetes. It also says these medicines are not for everyone with a high BMI. Our guide on how to get a semaglutide prescription walks through the steps. You should not take a GLP-1 injection if:
- You have a personal or family history of medullary thyroid carcinoma (MTC)
- You have multiple endocrine neoplasia syndrome type 2 (MEN 2)
- You have had a serious allergic reaction to the medicine
- You are pregnant or planning a pregnancy: NIDDK says weight management medicines may harm the fetus, and the Wegovy and Zepbound labels say to stop when pregnancy is recognized
Some situations need extra care. The labels say these medicines are not recommended with severe gastroparesis, and that taking them with insulin or a sulfonylurea can raise the risk of low blood sugar. If you take birth control pills, the Zepbound label says tirzepatide can make them less reliable for 4 weeks after you start and after each dose increase, so it advises switching to a non-pill method or adding a barrier method during those weeks. Tell your physician about any past pancreatitis, gallbladder, or kidney problems too. A physician who reviews your full history makes the final call.
Side effects and safety
The most common side effects are digestive: nausea, diarrhea, vomiting, constipation, and stomach pain. In STEP 1, nausea and diarrhea were usually mild to moderate and eased with time. Side effects tend to show up after starting or raising a dose.
Less common but serious problems listed on the labels include pancreatitis, gallbladder disease, kidney injury from dehydration, severe allergic reactions, and stomach contents entering the lungs during anesthesia or deep sedation, so tell anyone planning a procedure for you that you take one. The labels also carry a boxed warning: these medicines caused thyroid C-cell tumors in rodents, and it is not known whether they do in people. Severe stomach pain that will not go away, or vomiting that keeps you from holding down fluids, needs prompt medical care. For a fuller guide, read about semaglutide side effects.
How much do weight loss injections cost?
Prices vary with three things: which medication, which dose, and what the price covers. Some programs quote the medication alone and bill visits and lab work separately. Others bundle everything into one monthly price. Always ask what you will pay at your starting dose, at higher doses, and for anything extra.
MeliOra is a self-pay practice with published prices:
| MeliOra plan | Monthly price |
|---|
| Semaglutide, starting dose | $150/month |
|---|
| Semaglutide, higher dosing | $240/month |
|---|
| Tirzepatide, starting dose | $230/month |
|---|
| Tirzepatide, higher dosing | $320/month |
|---|
Each monthly price includes physician visits, lab work, the medication and supplies, and dose adjustments and check-ins, with no consultation fee. Your price follows your dose, so Dr. Espinet confirms it at your visit. If you are comparing affordable weight loss injections, compare the full monthly total; our breakdown of what medical weight loss costs in Lutz shows what to add up.

Frequently asked questions
What is the best injection for weight loss?
No single injection is best for everyone. In the SURMOUNT-5 head-to-head trial, tirzepatide led to more average weight loss than semaglutide over 72 weeks, but the right choice also depends on side effects, your health history, labeled uses, and cost. A physician should help you choose.
Do weight loss injections really work?
Yes, for many people, when combined with diet and activity changes. In large trials, adults on weekly semaglutide or tirzepatide lost much more weight on average than those on a placebo. Results vary, and NIDDK notes that most weight loss happens in the first 6 months.
Do you need a prescription for weight loss injections?
Yes. Semaglutide, tirzepatide, and the other GLP-1 medicines for weight management are prescription medicines, so a licensed clinician has to evaluate you and decide whether one is safe for you. At MeliOra, Dr. Espinet does that evaluation herself before prescribing semaglutide or tirzepatide.
Can you get weight loss injections if you do not have diabetes?
Yes. Wegovy, Zepbound, and Saxenda are labeled for weight management, and the STEP 1, SURMOUNT-1, and SURMOUNT-5 trials enrolled adults without type 2 diabetes. You still need a physician to confirm the medicine is right for you.
How long do you have to take weight loss injections?
There is no set end date. The weight-management labels describe these medicines as treatment to lose weight and keep it off long term, and NIDDK says you will probably regain some weight after you stop. Plan the next step with your doctor before stopping.
Are weight loss injections safe?
For adults who qualify, they can be used safely with a physician’s screening first and follow-up during treatment, but they are not right for everyone. Most side effects are digestive and ease with time, while serious risks such as pancreatitis and gallbladder disease are less common. People with a personal or family history of medullary thyroid carcinoma or MEN 2 should not take them.
How much do weight loss injections cost at MeliOra?
At MeliOra, semaglutide is $150/month at the starting dose and $240/month for higher dosing, and tirzepatide is $230/month at the starting dose and $320/month for higher dosing. Each monthly price includes physician visits, lab work, the medication and supplies, and dose adjustments, with no consultation fee and no travel fee.
