The short answerForehead Botox relaxes the frontalis, the muscle that raises your eyebrows, so horizontal forehead lines soften within days and look their smoothest at about two weeks. Typical plans use 10 to 20 units, or $120 to $240 at $12/unit, and the Xeomin label treats the frown lines at the same time to lower the chance of a heavy brow. Of our two Botox brands, the FDA has approved Xeomin for forehead lines; using Jeuveau there is off-label.
If you see rows of lines across your forehead when you raise your eyebrows, forehead Botox is the treatment most people mean. Botox for forehead lines can soften those creases while you keep a natural, expressive face, but the forehead is also an area where dose and placement shape how your brows sit afterward. This guide covers what causes horizontal forehead lines, what the evidence shows, units and cost at MeliOra, a before and after timeline, and the risks worth knowing. For the full treatment page, see Botox in Lutz, FL.
What causes horizontal forehead lines?
Horizontal forehead lines come from the frontalis, the broad muscle that runs up your forehead and lifts your eyebrows. The prescribing information for Jeuveau notes that expression lines form at right angles to the pull of the muscle beneath them. Because the frontalis pulls upward, its lines run across the forehead.
At first the lines show only when you raise your brows. With time they can stay when your face is relaxed. MedlinePlus, from the National Library of Medicine, names sun exposure as the greatest single factor in how skin ages, and age-related changes in connective tissue reduce the skin’s strength and elasticity. Botox addresses the muscle movement, not the sun damage. A quick mirror test tells you which kind of line you have:
- Raise your brows, then relax. If the lines disappear, they’re movement lines, the kind Botox is designed to soften
- If faint lines stay when your face is still, they’ve started to etch in. Botox can soften them, but it may not smooth them completely
Does Botox work for forehead lines? What the evidence shows
Botox with Jeuveau or Xeomin blocks the nerve signal that tells a muscle to contract. When the frontalis relaxes, the skin over it folds less, and the lines soften. The FDA has approved Xeomin to improve the appearance of moderate to severe horizontal forehead lines caused by frontalis activity, and its label doses the forehead together with the frown lines between the brows.
The Xeomin label reports two large trials, published in 2025, in which 730 adults with moderate to severe upper-face lines received Xeomin or a placebo. Forehead lines treated with Xeomin improved significantly more than placebo at day 30. Success had a strict definition: both the investigator and the patient had to rate the lines as none or mild, at least two grades better than before, with the brows fully raised. Depending on the trial and group, 58 to 67 of every 100 people treated with Xeomin met that bar for forehead lines, and none of the placebo group did.
A 2013 review of the forehead’s anatomy reached a practical conclusion: good results depend on fitting the dose and injection points to each person’s muscles, brow shape, and goals, because the frontalis works against the muscles that pull the brows down. In other words, there is no one-size forehead dose.
Jeuveau’s FDA approval covers frown lines only, so using Jeuveau on the forehead is off-label. The FDA explains that doctors may prescribe an approved drug for an unapproved use when they judge it medically appropriate, but that the agency hasn’t determined the drug is safe and effective for that use. Dr. Espinet tells you which brand she recommends, and why, before treatment begins.
How many units of Botox for forehead lines?
Forehead lines typically take 10 to 20 units, the range MeliOra publishes for this area. The Xeomin label dose is 20 units, given as 4 units at five points across the forehead, at least 2 cm above the bony rim of the eye socket, and paired with 20 units for the frown lines, for 40 in total. Jeuveau has no label dose for the forehead, and both labels say their units can’t be compared to or converted into units of another botulinum toxin product. For every area, see how many units of Botox you need.
| Forehead plan | Units (cost at $12/unit) |
|---|
| Forehead lines, typical range | 10 to 20 units ($120 to $240) |
|---|
| Xeomin label: forehead with frown lines | 40 units, 20 + 20 ($480) |
|---|
| Forehead, frown lines, and crow’s feet in one visit (Xeomin label maximum) | 64 units ($768) |
|---|
Where you land depends on the strength of the muscle, how low your brows sit, how far your lines spread, and how much movement you want to keep. A lighter plan leaves some lift when you raise your brows, and stronger muscles usually need more units.
Why forehead Botox is paired with the frown lines
Your brows sit where two sets of muscles balance. The frontalis lifts them. The muscles between and around the brows pull them down and together; the Jeuveau label describes the procerus and depressor supercilii pulling the brow downward. Relax only the lifter, and the muscles that lower the brow can win, which can leave your brows feeling heavy.
That’s why the Xeomin label says to treat forehead lines together with the frown lines, to minimize the chance of a drooping brow. It doesn’t give a dose for the forehead alone. Dr. Espinet decides whether to include the frown lines after watching how your brows move. For that area on its own, see Botox for frown lines.
One more point: forehead Botox isn’t a brow lift. Because it relaxes the muscle that raises them, the brows can sit a little lower. If a lift is your goal, a Botox brow lift is a separate plan that targets different points.
How much does forehead Botox cost?
At $12/unit, forehead lines at 10 to 20 units come to $120 to $240. The Xeomin label pairing of forehead and frown lines, 40 units, is $480. Jeuveau and Xeomin cost the same at MeliOra, there’s no travel fee anywhere in the service area, and you agree on your units and your total before treatment.
Your yearly total depends on your dose and how often you choose to retreat. For yearly budgets and the price of other areas, see what Botox costs in Lutz.
Forehead Botox before and after: a timeline
Forehead results build over days, not minutes. Here’s what a typical treatment looks like from the first hour to the next visit.
| When | What you may notice |
|---|
| Right after | Small raised spots at the injection points that settle within an hour or so. Mild redness or a small bruise is possible. |
|---|
| Days 2 to 7 | The effect begins, and lines crease less when you raise your brows. The Xeomin label puts the median onset at two to seven days after injection. |
|---|
| About 2 weeks | Close to the full effect, and the best time to compare before and after photos and judge your result. |
|---|
| Day 30 | When the Xeomin trials measured forehead results. |
|---|
| Months 3 to 4 | Movement gradually returns. The Xeomin label gives a typical duration of up to 12 to 16 weeks, and it varies from person to person. |
|---|
For fair before and after photos, take them in the same light, once at rest and once with your brows raised, before treatment and again at two weeks. Both labels say not to retreat more often than every three months. For more on onset, see how long Botox takes to work, and for how long results hold on the forehead, see how long Botox lasts.
What are the risks of forehead Botox?
In Xeomin’s placebo-controlled upper-face trials, 11 of every 100 people treated reported a side effect, compared with 8 of every 100 on placebo. In Xeomin’s repeat-treatment trials for upper-face lines, the most reported side effects were a small collection of blood under the skin at an injection point, headache, bruising, and a drooping brow, and they tended to become less common with later treatments.
- Heavy or low brows: the most specific forehead risk, and the reason the Xeomin label treats the frown lines too. In Xeomin’s placebo-controlled upper-face trials, a drooping brow affected fewer than 1 in 100 people
- A droopy eyelid: both labels give placement steps to lower this risk
- A raised or arched brow: in Xeomin’s frown-line trials, fewer than 1 in 100 people reported a raised eyebrow. Tell Dr. Espinet if your brows look uneven at two weeks
- Serious: in some cases, as with every botulinum toxin, the effect can spread beyond the treated area hours to weeks later. Get medical care right away for trouble swallowing, speaking, or breathing, new muscle weakness, or double or blurred vision
- Who should wait or skip it: anyone with an infection at the treatment site or a past allergic reaction to a botulinum toxin product. A neuromuscular condition such as myasthenia gravis may rule treatment out, and treatment is postponed during pregnancy and breastfeeding
If a brow or eyelid does droop, it’s generally temporary, though the Jeuveau label notes that side effects can last several months. Our guide to a droopy eyelid after Botox explains why it happens and what to do while it wears off.
Forehead wrinkles treatment: is Botox right for you?
Forehead Botox tends to suit you if your lines show when you raise your brows and you want them softer while still looking like yourself. The Xeomin label advises caution in a few situations: brows or eyelids that already sit low, a lot of extra eyelid skin, deep scarring, thick, oily skin, or marked asymmetry of the face. In those cases a lighter plan, different points, or no forehead treatment at all may be the better call, and Dr. Espinet will tell you which.
Sun protection matters at every age. MedlinePlus advises a good quality sunscreen whenever you’re outdoors, even in winter, plus protective clothing and a hat when needed. Botox doesn’t change skin texture, and treatments aimed at texture, such as resurfacing, aren’t on MeliOra’s menu.

Frequently asked questions
How many units of Botox do you need for your forehead?
Most people need 10 to 20 units for forehead lines. The Xeomin label dose is 20 units for the forehead, given together with 20 units for the frown lines, for 40 in total. Dr. Espinet sets your dose after watching how your brows move.
How much does forehead Botox cost?
At MeliOra, forehead Botox is $120 to $240 for 10 to 20 units at $12/unit. Forehead and frown lines together at the Xeomin label dose of 40 units come to $480. Jeuveau and Xeomin cost the same.
Is forehead Botox approved by the FDA?
Yes, for Xeomin. The FDA has approved Xeomin for moderate to severe horizontal forehead lines, and its label doses the forehead together with the frown lines. Jeuveau is approved for frown lines only, so using it on the forehead is off-label.
Can Botox get rid of deep forehead wrinkles?
Not always: Botox softens forehead lines made by movement, but lines that stay when your face is relaxed may soften without disappearing. In the Xeomin trials, 58 to 67 of every 100 people treated reached none or mild forehead lines at day 30 with the brows fully raised.
Does forehead Botox hurt?
Usually only briefly. The Xeomin label calls for a very fine needle and five points across the forehead, plus five more if the frown lines are treated too. In Xeomin’s upper-face trials, fewer than 1 in 100 people treated reported discomfort at the injection site as a side effect.
When will I see results from forehead Botox?
Many people see lines start to soften within the first week, and the result is close to its peak at about two weeks. The Xeomin label puts the median onset at 2 to 7 days, so wait two weeks to judge your before and after.
Can I get Botox on my forehead without treating the frown lines?
Sometimes, but it’s a judgment call. The Xeomin label describes treating the forehead together with the frown lines to lower the risk of a drooping brow, and doesn’t give a dose for the forehead alone. Dr. Espinet decides after watching how your brows move.
Will forehead Botox lift my eyebrows?
No, it isn’t meant to. Forehead Botox relaxes the muscle that raises your brows, so they more often sit slightly lower, though the Xeomin label lists a raised eyebrow as an uncommon reaction. A Botox brow lift is a separate treatment that targets different points.
